Charming Lakes Rehabilitation is seeking a motivated, detail-oriented, and compassionate
Admissions Coordinator to join our team. In this vital role, you will be the first point of contact for residents and their families—ensuring a seamless admissions experience while building strong relationships with referral sources and the surrounding community.
Key Responsibilities
- Identify and monitor operational areas impacting admissions; report findings and assist with process improvements
- Support marketing and outreach efforts by attending community events and developing referral relationships
- Educate families and guests on facility policies, visitor guidelines, and resident care procedures
- Coordinate admissions with hospitals and referral sources to ensure smooth and timely transitions
- Build and maintain strong relationships with hospitals, physicians, and community partners
- Verify financial and insurance information prior to admission and communicate details to the interdisciplinary team
- Maintain up-to-date knowledge of Medicare, Medicaid, and Managed Care processes
- Track, analyze, and report admissions data, trends, and performance metrics
Qualifications
- Minimum of 2 years of admissions experience in a Skilled Nursing Facility
- Strong understanding of Medicare, Medicaid, and insurance processes (including PDPM, HMO guidelines, and RUGS II & III)
- Excellent communication, organizational, and interpersonal skills
