Job Summary and Qualifications

What you will do in this role:

• Assist with facilitating patient discharges.
• Identify and report barriers to discharges.
• Provide administrative support in the case management functions.

What qualifications you will need:

• Associates degree preferred
• Minimum of one-year experience in a health care setting required.
• Medical terminology required
• Healthcare professional (PCT, CNA, EMT) preferred
