Moorings Park is looking for a Community Liaison. The Community Liaison provides clients and referral sources with agency related information through personal visits, participation in community health related events, membership in health and other related community organizations, and networking with other medical and health care communities. The Community Liaison is an expert in communicating the services provided by the Home Health Agency to develop business relationships and market the services. The Community Liaison is responsible for insuring and coordinating quality and safe delivery of home health services through the intake process of collecting client data prior to admission
Contributions:
- Participates in developing a marketing plan and actively pursues referral sources, as well as facilitates on-going relationships with various medical and health communities.
- Works to develop an open, positive rapport and relationships with community resource’s affiliated with health care services.
- Maintains high visibility and availability while in the office via telephone to physicians, referral sources, community resources, clients, and staff.
- Responds to inquiry calls from hospital, families, physician, and referral sources. Ensures that responses to inquiries are handled properly, timely and with appropriate follow-up.
- Makes calls in person to financial, legal and medical professionals, senior organizations, appropriate special interest groups, facility discharge planners, and other community contacts.
- Maintains relationships to advise community organizations and referral sources of home health services available to efficiently meet the daily needs and inquiries of the community at large.
- Develop and maintains a database for tracking medical and community resources and referral sources.
- Comply with applicable federal, state, and local laws, rules, and regulations; maintains knowledge of and follows all company policies and procedures.
- Obtain knowledge and demonstrate the principles of Moorings Park, person directed care and successful aging philosophies and their inherent core values of commitment, respect, compassion, dedication, teamwork, and quality. Team member will support, actively participate, and act in accordance with the principles of the Moorings Park service culture and be familiar with and fluent in the language and service pillars of Moorings.
Job Requirements:
- Excellent oral and written communication skills in the English language required
- During emergencies or natural disasters may be required to be on duty for the duration of the disaster.

- Associate or bachelor’s degree, preferably in marketing or related field.
- Previous community relations and marketing experience in healthcare preferred.
- Required to use own vehicle to travel off campus. A current Florida driver’s license, clean driving record and proof of insurance are required.
- Microsoft office or equivalent experience preferred
