A-Line Staffing is now hiring a Case Management Coordinator (Bilingual – Spanish/English) in Coral Gables, FL. This position offers competitive pay and the opportunity to support members through care coordination in a community-based and remote setting.

Case Management Coordinator Compensation

Pay Rate: $34.13/hour

Case Management Coordinator Highlights

• Location: Coral Gables, FL (Must reside in Miami-Dade County – 33134, 33146, 33133, 33143, 33156, 33114)
• Assignment Type: Contract
• Work Setting: Remote with field travel (50–75%)

Schedule

Monday – Friday: 8:00 AM – 5:00 PM

Additional Information

• 50–75% travel within Miami-Dade County (home visits, assisted living facilities, nursing homes)
• Mileage reimbursement available
• Training conducted remotely via Microsoft Teams (~1–2 weeks)
• Must be fluent in Spanish and English (speaking and writing)
• Non-nursing role

Case Management Coordinator Responsibilities

• Coordinate case management activities for Medicaid Long Term Care members
• Conduct telephonic and face-to-face member assessments
• Develop, implement, and monitor individualized care plans
• Collaborate with providers, caregivers, and community resources
• Support members with care coordination, education, and wellness planning
• Identify and escalate quality of care concerns
• Assist with prior authorizations, service coordination, and discharge planning
• Maintain accurate documentation and compliance with regulatory guidelines
• Encourage member engagement through coaching and motivational interviewing

Case Management Coordinator Requirements

• Bilingual (Fluent in Spanish & English – required)
• Bachelor’s degree in Social Work or related field
• Minimum 1+ year of experience in behavioral health or long-term care
• Ability to travel 50–75% locally within Miami-Dade County
• Strong communication, organization, and multitasking skills
• Basic to intermediate Microsoft Office proficiency

Preferred Qualifications

• Case management or discharge planning experience
• Managed care experience
• Experience working with medically complex populations
