A Registered Nurse (RN) Case Manager is needed in Fort Pierce, FL.

• The salary range is $75,000 to $95,000 per year
• $600 Monthly Fuel Stipend if 40 Miles from Facility

Responsibilities

• Schedule: Monday-Friday (8am – 5pm)
• Caseload: 8-13 Patients per Day
• Call: 1 in 6 or 7 Weeks
• Provide quality care through implementation and evaluation of individualized patient care plans, discharge planning and complex outpatient needs.
• Perform prospective, concurrent and retrospective evaluation of clinical information to establish medical necessity, appropriateness of services and cost effectiveness.
• Serve as patient and family advocate during hospital stay and outpatient status.
• Conduct in-home or hospital evaluations in some circumstances.

Requirements

• Associate’s Degree in Nursing
• Registered Nurse License in the State of Florida
• Certification in Case Management (or the ability to obtain within one year of employment)
• Minimum of 2 years of experience as a Medical Case Manager
• Experience as a Behavioral Health / Psych RN
• Current BLS certification
