The Role: In a Nutshell

We are seeking a dynamic and experienced Registered Nurse (RN) to join our team as the Chronic Care Management Clinical Advisor. In this role, you will work closely with the Director, Clinical Operations to provide a blend of internal clinical support and product development, and serve as an external clinical resource to engage prospects and demonstrate our clinical abilities. The ideal candidate will have a strong clinical background, excellent leadership skills, and a passion for improving patient outcomes through innovative healthcare solutions.

Responsibilities:

External:

  • Serve as Clinical SME to prospective and current clients, assisting with demos and relaying our clinical benefit.
  • Engage with existing clients to communicate clinical initiatives.
  • Service recovery/ escalation issues of a clinical nature, engaging clients in an issue resolution process
  • Participate in Quarterly Client Advisory Board Meetings

Internal:

  • Serve as the head clinical advisor to Care Coordinators (CCs):
    • Ensure the delivery of high-quality Chronic Care Management and Remote Patient Monitoring.
    • Serve as a physical health clinical subject matter expert, providing guidance and support on clinical matters.
    • Oversee internal and external escalations related to patient care, including but not limited to medication management, care plan adherence, and patient engagement.
  • Internal clinical initiatives:
    • Help define clinical content for our programs
    • Collaborate with cross-functional teams such as Product, to develop and implement clinical protocols, guidelines, and best practices to optimize patient outcomes.
    • Participate in strategic planning and decision-making processes to drive the growth and success of the clinical operations department.
  • Develop and deliver educational programs, in-services, and training sessions to clinical staff on new procedures, equipment, clinical competencies, and best practices.
  • Identify educational needs within the clinical team and create resources to address them.
  • Participate in the development, implementation, and evaluation of clinical policies, procedures, and protocols to ensure alignment with current standards of care and regulatory requirements (e.g., Joint Commission, CMS, State Boards of Nursing).
  • Contribute to root cause analyses of adverse events and develop corrective action plans.
  • Monitor clinical outcomes and trends, utilizing data to inform quality improvement initiatives.

Requirements:

  • Active Registered Nurse (RN) license; Bachelor of Science in Nursing (BSN) preferred.
  • Minimum of 5 years of clinical experience in a healthcare setting, with at least 2 years of experience in Virtual Care Management or Transitions of Care services for the elderly population. Chronic Care Management or Remote Patient Monitoring experience preferred.
  • CCM Certification preferred or will obtain within 1 year of hire.
  • Strong clinical assessment and critical thinking skills, with the ability to make sound clinical judgments in complex situations.
  • Experience in managing and leading clinical teams, including providing clinical supervision and mentorship.
  • Proficiency in electronic health records (EHR) and other healthcare technology platforms.
  • Excellent communication and interpersonal skills, with the ability to effectively interact with patients, caregivers, and multidisciplinary healthcare teams.
  • Knowledge of Medicare and Medicaid guidelines, quality measures, and regulations related to chronic care management and remote patient monitoring.
  • Demonstrated ability to work independently, prioritize tasks, and meet deadlines in a fast-paced environment.
