Quality Improvement Specialist
RTD Talent Solutions is currently representing one of Florida's premier networks of primary care centers and physicians that provides accessible, patient-centered care across a number of locations that is currently seeking a Quality Improvement Specialist for their Tampa, FL location. This is a full-time, on-site, permanent role with excellent compensation and benefits.
As the Quality Improvement Specialist, you would be responsible for driving key quality initiatives by working closely with patients, clinical teams, and health plan partners to close care gaps, strengthen STARs/HEDIS performance, and improve overall patient health outcomes.
Key job duties, among others:
- Execute targeted patient outreach — including in-office visits, phone calls, and secure messages — to resolve open HEDIS care gaps.
- Plan and lead focused quality campaigns such as TRC, immunizations, A1c control, hypertension management, and cancer screening initiatives.
- Track and follow up on emergency department utilization and post-discharge/inpatient transitions.
- Accurately upload compliant quality outcomes and documentation to payer portals.
- Deliver training and ongoing education to staff on HEDIS measures, quality standards, and documentation requirements.
- Collaborate with clinical and operational leaders to develop action plans addressing performance gaps and compliance issues.
- Participate in multidisciplinary care coordination meetings to ensure alignment on quality priorities.
- Support community outreach and patient-engagement events, including occasional Saturday activities.
- Travel between practice sites (approx. 50%) to provide education, conduct audits, and support outreach efforts.
