Job Summary
We are seeking a per diem Physical Therapist to join our team! As a Physical Therapist, you will visit patients in their homes, discuss their treatment needs and goals, and develop individualized, comprehensive therapy plans. You will collaborate closely with the patients healthcare team, arrange appropriate follow-up care, educate patients on recommended lifestyle changes, and provide a positive and supportive presence.
The ideal candidate is compassionate, has strong communication and interpersonal skills, and is comfortable managing multiple cases at a time.
- Meet with patients from diverse backgrounds and compassionately discuss their health concerns and therapy needs
- Conduct evaluations to establish a baseline for each patient
- Develop individualized therapy plans to help patients achieve their mobility goals
- Collaborate with the patients care team to ensure effective follow-up care
- Maintain accurate and detailed documentation of patient therapy and progress
- Active state Physical Therapy license
- Strong communication and interpersonal skills
- Excellent organizational skills
- Attention to detail
- Ability to remain positive, patient, and compassionate over extended periods of care
- Evaluation: $80.00
