Bilingual Healthcare Case Manager (168521)
Pay: $34.13 per hour
Schedule: Monday – Friday, 9:00 AM – 5:00 PM
Hours: Full Time / 40+ hours
Pay Frequency: Bi-Weekly
Benefits: Health, Dental, Vision, Life Insurance, Short-Term Disability; 401(k) with company match (after 1 year of service)
Location: Coral Gables/Doral, FL (Remote with travel; mileage reimbursed)

Bilingual Healthcare Case Manager Responsibilities
- Coordinate case management activities for Medicaid Long Term Care/Comprehensive Program enrollees
- Assess, plan, implement, and coordinate case management activities to evaluate members' medical needs
- Develop proactive courses of action to enhance member wellness through integrated services
- Identify and escalate quality of care issues through established channels
- Engage members in lifestyle and behavior changes using motivational interviewing skills
- Provide coaching and support to empower informed healthcare decision-making
Bilingual Healthcare Case Manager Requirements
- High School Diploma or GED
- Bachelor’s degree in social work or a related field
- Case management experience required (Long Term Care preferred)
- Fluent in English and Spanish
- Ability to travel 50–75% within region
