Avail Health & Behavioral Solutions is seeking a motivated and compassionate Community Health Worker (CHW) to support individuals and families in navigating healthcare systems, accessing community resources, and improving overall health outcomes.
The CHW will serve as a bridge between clients, providers, and community services to reduce barriers to care and promote self-sufficiency.
This is a field-based role requiring strong communication, organization, and advocacy skills.
Key Responsibilities
- Conduct outreach and engage individuals in the community
- Perform needs assessments and develop action plans
- Assist clients with accessing medical, behavioral health, housing, food, and transportation resources
- Support care coordination with physicians, case managers, and service providers
- Provide education on chronic disease management, medication adherence, and preventive health
- Assist clients with scheduling appointments and completing applications
- Maintain accurate documentation and case notes
- Participate in interdisciplinary team meetings
- Track progress toward client goals and outcomes
Preferred Skills
- Experience working with underserved populations
- Knowledge of housing navigation, employment resources, or re-entry support
- Bilingual a plus
- Experience with electronic documentation systems
