What You’ll Do

• Evaluate patients and develop individualized occupational therapy care plans
• Help patients improve independence with daily living activities and mobility
• Assess strength, coordination, safety, adaptive equipment, and home environment needs
• Provide therapeutic interventions that improve function and quality of life
• Collaborate with interdisciplinary teams to coordinate exceptional patient care
• Complete documentation and clinical records accurately and timely
• Educate patients and families to support long-term success at home

Qualifications

• Active Occupational Therapy License
• 1 year of Occupational Therapy experience preferred
• CPR Certification
• Valid Driver’s License & Auto Insurance
• HomeCare HomeBase experience preferred

Physical Requirements

• Ability to stand, walk, kneel, squat, and move frequently throughout the day
• Ability to lift, carry, push, or pull up to 100 lbs.
