Director of Social Services
- Develop and implement department policies and procedures that align with regulatory requirements and organizational standards.
- Recruit, manage, and retain a team of qualified social services professionals to maintain appropriate staffing levels.
- Coordinate staff training, in-services, and continuing education opportunities.
- Oversee timely and accurate data collection and reporting to the Administrator.
- Participate in facility-wide committees and interdisciplinary care team meetings.
- Supervise the admissions process and ensure a smooth, supportive transition for new residents.
- Act as the Discharge Planning Coordinator, identifying and addressing barriers to safe and timely discharges.
- Serve as the Grievance Officer, managing and resolving resident concerns in a timely and compassionate manner.
- Collaborate with hospitals, health centers, and community agencies to ensure continuity of care.
- Promote awareness and enforcement of the Resident Bill of Rights.
- Monitor and evaluate the quality and effectiveness of social work services.
- Ensure compliance with healthcare standards and assist other departments with regulatory documentation.
- Prepare and submit monthly, quarterly, and annual reports as required.
- Conduct internal audits and generate reports related to care quality and departmental performance.
- Plan, organize, and supervise all administrative and operational activities within the Social Services Department.
- Minimum of 2 years' experience in a medical or healthcare setting
- Strong leadership and communication skills
- In-depth knowledge of state and federal healthcare regulations
