Licensed Practical Nurse (LPN) - Home Health
Job Summary:
The LPN provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). The role focuses on ensuring patient safety, comfort, and adherence to care plans.
Responsibilities:
- Deliver direct patient care (vital signs, wound care, medication administration).
- Follow individualized care plans and physician orders.
- Monitor and report changes in patient condition.
- Assist with activities of daily living as needed.
- Maintain accurate and timely documentation.
- Educate patients and families on care and health management.
Qualifications:
- Active LPN license in the state of Florida.
- Current CPR certification.
- Home health or related experience preferred.
- Strong communication and organizational skills.
Work Environment:
Provide care in patient homes; drive to their homes required.
