The Per Diem Nurse at Aiding With Care Health Services, INC is essential in maintaining the health and wellness of our clients through periodic assessments and routine care. Duties and Responsibilities

• Complete in-home assessments and client intakes to understand individual care needs.
• Develop personalized and detailed plans of care in coordination with other healthcare professionals.
• Implement care plans effectively, adjusting to the client’s needs and health changes over time.
• Document clinical findings and updates accurately and in a timely manner in the patient's medical record.
• Communicate effectively with clients, their families, and other members of the healthcare team.
• Ensure compliance with all relevant home care standards, policies, and regulations.
• Participate in ongoing education and training sessions to keep up-to-date with the latest in nursing practices and standards.
• Attend and contribute to team meetings to discuss care plans and client progress.
• Provide emotional support and guidance to patients and their families, helping them navigate the healthcare process.
• Maintain professionalism and a high degree of confidentiality at all times.
• Manage supplies and resources efficiently, ensuring availability for each home visit.

Requirements

• Active Florida Registered Nurse license.
• Previous home care or home health experience is preferred.
• Strong assessment and documentation skills.
• Ability to travel and provide on-site nursing care within Broward County.
• Demonstrated ability to develop and implement comprehensive care plans.
• Excellent communication skills, both verbal and written.
• Adept at multitasking, prioritization, and managing time efficiently.
