- Educates members on their care team and assists with coordination of care across the continuum, including scheduling appointments and referrals.
- Participates in daily operations of the practice, including answering phone calls, assisting with front desk inquiries, and maintaining the clinical space.
- Communicates through various digital channels to support patient care. Performs initial health screens and intake for new members.
- Obtains and reviews old medical records to ensure comprehensive patient history. Identifies care gaps and orders necessary screening tests.
- Confirms appointments and provides instructions to patients.
