• 40 hours per week, working days, some weekends roughly. Will discuss further in interview. 
• Pay is on a point system, overall hours will vary based on cases and types of visits. Salary is based on location and skill level.

Job Responsibilities:

• Provide physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.
• Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient’s ability to perform activities of daily living.
• Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of mobility.
• Reports to the physician and designated agency representative patient’s reaction to treatment and any changes in the patient’s condition. 

Required Skills & Experience: 

• One to two (1-2) years of experience.

Preferred Skills & Experience: 

• Minimum of one (1) year of experience in home health care. 

Required Education: 

• DPT from CAPTE accredited program. 

Required Certifications & Licensure: 

• Licensure as a physical therapist in the state of Florida. 
• BLS from the American Red Cross or American Heart Association.
