Nurse Navigator-Lee Physicians Group-Structural Heart
Lee Health is actively seeking a dedicated Nurse Navigator to join our Structural Heart program within the Lee Physicians Group. This role is pivotal in providing specialized nursing care and coordination of services for patients undergoing treatment for structural heart disease. The ideal candidate will work closely with cardiologists, surgeons, and a multidisciplinary team to ensure a seamless continuum of care from diagnosis through follow-up, optimizing patient outcomes and enriching patient and family education.
