JOB SUMMARY:
Provides support services to consumers residing in the community or in a permanent housing setting of a supported housing program as a part of Living Independently, Connecting Communities. Assist in securing appropriate Services including by not limited to vocational support, medical support, finding community supports and services, assisting with medication and psychiatric follow up and providing recovery services. Services are designed to promote independent living and mental health stability.
PRIMARY JOB FUNCTIONS:
- Assesses and screens consumers as required. Works directly with consumers to monitor and document mental health status, medication compliance, progress and performance toward independent living. Provides as necessary, appropriate crisis management services. Provides individual counseling as needed/desired. Obtains financial resources as necessary. Plans, conducts and documents sessions in accordance with policy and procedure.
- Works with clients to develop a comprehensive service plan to stabilize their housing. Implements service plans for each assigned client; updates plan every six months.
- Documents all client services provided within a forty-eight hour period and maintains productivity standards to the level set by the department
- Provides direct/indirect assistance, guidance and instruction in tasks related to the maintenance of independent living skills. Documents required reports and basic living skills service provision in accordance with policy and procedure. Provides one-to-one counseling and monitors consumer progress.
- Promotes safe and clean residences, ensuring needed repairs are reported and completed. Locates potential and suitable community housing and negotiates with landlords to assure resolution of problems and disputes.
- Transports consumers as needed in own vehicle or agency vehicle when available. Maintains required coverage on their vehicle and notifying their insurance company that they use their vehicle for work.
- Maintains regular communication and coordinates client care with team members and other service providers. Participates, as needed, in group meetings/professional staffing to ensure appropriate delivery of services.
- Assists consumers with completion of paperwork for available community resources (i.e. food stamps, social security benefits, vocational rehabilitation, HUD, etc.). Develops supports and/or services as needed or desired by consumers that promote more effective independent living.
- Maintains necessary tenant files, records and other documentation in accordance with internal and/or external requirements. Prepares written narratives of service plans and interventions.
- Assists consumers with resolution of disputes/problems, ensuring proper procedures are employed regarding the handling of grievances or complaints.
- Makes referrals to providers as appropriate. Reports as appropriate, all unusual events, activities and incidents.
- Communicates program services to internal and external case management and DCF personnel.
- Performs monthly apartment inspections and fills out the accompanying documentation if consumer lives in a Boley-owned apartment.
EDUCATION AND EXPERIENCE:
Bachelor's degree is required. Human Services related field preferred. Education must be from an accredited school, college, or university.
Minimum of one year experience working with the severely mentally ill, community services and resources experience with property management/ tenant--landlord issues preferred.
Knowledge of counseling techniques and mental illness.
SKILLS AND ABILITIES:
Good skills and demonstrated abilities in verbal and written communications, coordination/cooperation, follow-through, interpersonal relations, attention to detail, use of basic language and math, following verbal and written procedures and instructions, recording information orally and in writing, internal and external public relations, working independently, verbal and physical crisis intervention techniques, planning, problem solving, negotiating, flexibility to change, decision making, scheduling own and others' work, counseling techniques and evaluation techniques, organizing own and others' work and such others that are usually and customarily considered necessary for satisfactory completion of essential duties.
WORKING CONDITIONS:
Job environment includes indoors, outdoors and office work. May be exposed to such environmental conditions as fumes, odors, dust, gasses, uncomfortable cold/heat, variable temperatures and uncomfortable noise; exposure to a potential hazard may occur when handling household cleaning chemicals; physical hazard may exist when dealing with unstable or difficult client/tenant. Overtime may be required from time to time. May require overnight and/or out of town travel, on-call duty from time to time.
SPECIAL REQUIREMENTS:
Valid Florida driver's license.
Personal transportation vehicle and vehicle insurance required, with vehicle work coverage insurance strongly encouraged.
Must pass a level II background and drug screen.
