We are seeking an RN/LPN Clinical Coordinator/Manager to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients.

The RN/LPN Clinical Coordinator provides clinical and administrative support to ensure high - quality patient care and efficient clinical operations. This role coordinates patient services, supports nursing staff, assists with care planning, and ensures compliance with clinical standards, policies, and regulatory requirements. The Clinical Coordinator serves as a key liaison between patients, providers, and clinical staff.

Responsibilities:

• Administer nursing care to ill, injured, or disabled patients
• Diagnose and establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate patient medical records
• Review care notes each shift to ensure accuracy and continuity of care
• Collect, track, and communicate physicians' orders to ensure timely implementation
• Serve as liaison between clinical staff, providers, clients, and families to support coordinated care
• Collaborate with interdisciplinary team members to support efficient clinical operations
• Process insurance eligibility verifications and obtain required prior authorizations, ensuring accurate coverage validation and timely approval to support uninterrupted patient services
• Accept and review electronic client referrals from hospitals and nursing facilities

Qualifications:

• Previous experience in nursing or other medical fields
• LEAD mentality a MUST
• Experience in long-term care and home health
• Familiarity with medical software and equipment
• Ability to build rapport with patients
• Strong problem-solving and critical thinking skills
• Ability to thrive in a fast-paced environment
