CHOSEN FAMILIES COUNSELOR
JOB DESCRIPTION
POSITION FUNCTION/OVERVIEW:
The counseling services provided by a counselor working in the Chosen Families program will work with post
adoptive children and families in Manatee County. This position is responsible for providing crisis intervention, individual/family counseling and group counseling with the adoptive children and their families. The position helps youth and families identify problems and find solutions through assessment, planning, and counseling.
This is a Counselor position that will work with a team of other professionals in order to provide collaborative
adoption preservation and support services. Routine supervision is provided with a less detailed review as
experience is gained. Initiative is encouraged. This position maintains an active caseload and has regular contact with clients. Services are generally provided at an assigned location but may be required at off-site locations as well.
This Counselor position will offer a hiring bonus of $1,000 at time of signing and after nine months of employment, another bonus of $1,000 will be issued.
QUALIFICATIONS:
Education requirements:
A License (LMHC, LCSW, or LMFT) and Master’s Degree in the field of clinical social work, mental health
counseling, psychology or related field.
Or License eligible (RMHCI, RMFTI or RCSWI) with the Florida Department of Health with at least 5 year
experience working with dependency and/or post adoptive families.
Experience requirements:
A minimum of two years of direct experience working with children and families who are victims of physical
abuse, sexual abuse and neglect and are emotionally disturbed.
Experience using the most current Diagnostic and Statistical Manual (DSMar), the Diagnostic Classification
of Mental Health and Developmental Disorders of Infancy and Early Childhood (DC: 0-3) and their
respective systems of crosswalk to the International Classification of Diseases, Clinical Modification (ICD).
Knowledge of the mental health and support services available in the community.
Ability to assess familial problems and assist in developing intervention strategies.
Other requirements:
Must successfully complete the Adoption Competency Training from the Rutgers School of Social Work
Must complete the “Charting the Course” facilitator training and 6 hours of adoption competency training
within 90 days of hire.
Knowledge of social services delivery system.
Ability to handle sensitive and confidential information with discretion.
Ability to communicate in clear, effective manner both written and verbally.
Ability to develop and maintain effective working relationships.
Ability to work five day work week with minimum 3 evenings and or weekend hours as assigned.
Ability to develop and maintain effective working relationships with individuals from a variety of cultural and
ethnic backgrounds. Must demonstrate awareness and acceptance of cultural differences of clients, staff
and community contacts.
RESPONSIBILITIES:
Complete all agency required training, including but not limited to HIPAA, Medicaid orientation, Risk
Management, CFARS, Deaf and Hard of Hearing, and Cultural Competence.
Comply with all regulations as outlined in the Medicaid Community Behavioral Health Services Coverage
and Limitations Handbook.
Complete and submit required clinical documentation to Counseling Supervisor within the established time
frames, including ensuring documentation is entered into the client’s medical record in compliance with
agency policy and procedure (MH-09).
Respond to communication about client treatment needs and/or documentation from Counseling Director
within 24 hours.
Participate in quality improvement and quality assurance measures as necessary.
Comply with all current agency, program and regulatory (e.g., HIPAA) policies and procedures.
Act in accordance with professional and ethical guidelines.

Counseling Job Description
Receive referrals and screen potential clients and provide counseling services to all assigned cases at an
appropriate level based on identified client need.
Conduct family assessments and prepare individualized treatment/service plans.
Provide crisis counseling to youth and families in office, home or school.
Conduct individual, family and group counseling as assigned.
Provide and facilitate inter-agency and community referrals, case management, client advocacy and
discharge planning as needed.
Perform all administrative and record keeping tasks associated with client case records in designated time
frame.
Must be available to work evening and weekends shifts to meet program needs.
Maintain average utilization rate of 60% for assessments and on-going counseling community based.
Attend case consults regularly and provide a written plan for discussion of clients and families.
Attend staff meetings regularly and participate in peer review activities on a rotating basis.
Lead case consultation in the absence of the Supervisor and Program Director.
Participate in community awareness and outreach activities.
Participate in various community meetings related to adoption preservation.
All other duties as assigned.
PHYSICAL DEMANDS/WORK CONDITIONS:
This position regularly requires the ability to sit for sustained periods of time and to effectively communicate with and assess clients, using verbal and nonverbal cues. This position also requires the ability to operate office equipment. Duties are performed primarily in an office setting. On occasion will require the use of personal automobile to travel to client’s homes for in-home sessions and could be exposed to changing weather. The noise level is moderate. Occasional local travel required for meetings, training, and to provide support between offices and to families.
