What You’ll Do

Patient Care & Treatment

• Provide physical therapy services in accordance with a physician’s written plan of care

• Evaluate patients’ functional status, mobility, strength, gait, and range of motion

• Assess the home environment for safety and rehabilitation potential

• Develop and implement individualized treatment plans

• Determine therapy priorities and adjust care as needed

• Monitor patient progress and report changes or responses to the physician

• Train patients in the use of prosthetic devices when applicable

Education & Collaboration

• Instruct patients and families/caregivers in therapy techniques and the use of therapeutic equipment

• Participate in case conferences and interdisciplinary care planning

• Provide consultation to families/caregivers as needed

• Deliver in-service education to agency personnel when appropriate

• Participate in peer consultations

Documentation & Coordination

• Prepare and submit timely clinical notes and progress summaries

• Participate in discharge planning

• Identify needs for additional home health services and initiate referrals

• Ensure necessary equipment and services are arranged and available

Leadership & Supervision

• Supervise physical therapy aides

• Provide instruction, oversight, and evaluation of aide services every 14 days

• Management experience not required

What We’re Looking For

• Active Physical Therapist (PT) license in Florida

• Current CPR certification

• Strong written and verbal communication skills

• Reliable transportation

• Home health experience preferred
