Job Summary: Physical Therapist Outpatient (PT OP) New Port Richey, Florida. This role supports the evaluation and treatment of outpatient patients by developing and implementing individualized treatment plans aligned with patient needs, goals, and physician direction. The position includes comprehensive patient assessments, documentation, and collaboration with the care team to support functional improvement and safe discharge planning. Candidates are required to bring their license on the first day.
Job Details:
- Schedule: Days; 8 hours per shift; minimum 40 hours per week.
- Start Date: 07/06/2026.
- Required Qualifications: Degree in Physical Therapy from an accredited school; Active/applicable license for the State where work will be performed; CPR Certification; 1+ year of outpatient clinical experience.
- Preferred Qualifications: Prior successful assignment with the agency; prior successful assignment with the organization.
- Responsibilities:
- Complete comprehensive evaluations of assigned patients and use clinical reasoning to develop treatment plans and goals focused on progressing patients toward discharge within an established length of stay.
- Ensure care is provided under signed, valid physician orders and within insurance approval, as indicated and necessary for clinical care.
- Complete medical record documentation in accordance with state, federal, and intermediary guidelines and clinic policies, including documentation, goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry using approved formats.
- Delegate and direct discipline-specific therapy services for assigned patients, supporting functional treatment programs that promote return to community activities.
- Participate in student programming as assigned, providing an appropriate learning environment and supervision, including development of learning objectives and completion of student evaluations.
- Create and maintain an individualized professional development plan with clinical management, pursuing professional growth through continuing education, self-study, and involvement in professional activities and organizations.
- Adhere to the discipline-specific State Practice Act and other applicable regulatory agencies, maintain professional licensure in good standing, and complete state-mandated continuing education.
- Perform duties ethically in accordance with the discipline-specific professional code of conduct.
- Follow the organizations performance improvement methodology, including awareness of initiatives and follow-through with action plans, and provide feedback through staff meetings, focus groups, or direct supervisor input as appropriate.
- Demonstrate professional behaviors consistent with the organizations mission and core values, including compliance with the organizations Code of Conduct and confidentiality requirements regarding patient and organizational information.
