(LPN) Virtual Nurse Coordinator

Role Overview:
As a Patient Care Coordinator, you’ll guide patients through their health journeys via monthly phone outreach, individualized care plans, and coaching on wellness, nutrition, and treatment goals.
Requirements:
- Active Compact LPN license.
- 2+ years clinical experience (care coordination preferred).
- Comfortable with EMRs, Microsoft Office, and other digital tools.
- Strong communication and problem-solving skills.
- Must use a desktop or laptop for assessments and work (tablets/phones not compatible).
Schedule & Pay:
- Full-time, 40 hrs/week | Monday-Friday, 8 AM-6 PM (patient’s time zone).
- Orientation/Training (first 2 months): $20/hr.
- Monthly bonus potential up to $525.
- Referral bonuses up to $1,000.
