Minimum Requirements

• Graduate of an accredited Physical Therapy program (Bachelor‘s, Master’s, or Doctoral degree).
• Active Physical Therapist license in Florida prior to the start of the assignment.
• Minimum of 1 year clinical experience as a Physical Therapist. Home health experience strongly preferred.
• Current CPR/BLS certification from AHA or ARC.
• Proficiency with electronic documentation systems and mobile technology.
• Valid driver’s license, reliable transportation, and active auto insurance.
• Ability to work independently in the field with strong clinical judgment and time management skills.
• Willingness to travel within the assigned coverage area and adapt to different branch operations.
• Client-specific requirement: Must be able to meet a 40-hour weekly guarantee and be available for on-call/weekend requirements as requested by the client.
• Preferred: Prior home health travel assignment experience and strong knowledge of Medicare Part A home health regulations and skilled documentation requirements.

• Coverage area: The Villages and surrounding areas. Candidate must travel between patient visits; mileage is reimbursed.
• Weekly hours: 40 hour weekly guarantee.

Role Summary and Responsibilities

• Provide comprehensive physical therapy evaluations and ongoing reassessments in patient homes in accordance with physician orders and agency policies.
• Develop, implement, and revise individualized plans of care focused on mobility, strength, balance, endurance, pain management, and functional independence.
• Deliver skilled interventions including therapeutic exercise, gait and transfer training, balance activities, stair training, and home safety education.
• Educate patients and caregivers on home exercise programs, fall prevention, use of assistive devices, energy conservation, and safety strategies.
• Document evaluations, visits, progress notes, and discharges accurately and timely in the agency EMR.
• Coordinate care with physicians and the interdisciplinary team and report changes in patient condition or safety concerns promptly.
• Manage a mobile caseload efficiently while meeting productivity and documentation expectations.

Professional and Compliance Requirements

• Adhere to CMS Conditions of Participation, infection control standards, and agency policies.
• Comply with agency background and onboarding requirements.
• Dress code: agency permits any scrub color.
