Contract Length: 20 weeks with possible extension
Shift: Day shift, 8:00 AM – 5:00 PM (Monday–Friday, alternating weekends)

Job Description:
We are seeking an experienced Home Health Registered Nurse to provide skilled nursing care to adult and geriatric patients in their homes. The RN will perform 6–8 skilled visits per day, focusing on cardiopulmonary assessments, wound care, IV therapy, and chronic disease management. This role requires strong clinical skills, independence, and the ability to deliver compassionate care in a home setting.

Responsibilities:

• Provide skilled nursing visits for patients with general medical and high-risk diagnoses (HF, COPD, MI, pneumonia, stroke, post-op care)
• Perform cardiopulmonary assessments, wound care (including wound vac management), IV therapy, and phlebotomy
• Manage PICC lines, midlines, ports, drains, and catheters
• Administer medications (IM, SQ) and perform lab draws
• Educate patients and families on disease management and care plans
• Document care using BayCare systems (Cerner preferred, Ace Timesheet, BayCare Forms)
• Collaborate with interdisciplinary teams including PT, pharmacy, social services, and interpretation services

Requirements

Qualifications:

• Active RN license (FL or compact) required
• Minimum 1 year of skilled home care experience (private duty experience not accepted)
• BLS certification required
• OASIS training and experience required
• Strong IV and wound care skills
