Position Overview
The Clinical Team Manager (CTM) oversees and leads a team of nurses and clinical staff, ensuring exceptional patient care, regulatory compliance, and operational excellence across home health services. This role blends clinical expertise, leadership, and strategic coordination to deliver high-quality, patient-centered care.

Key Responsibilities

• Lead, supervise, and develop a team of clinical case managers (RNs and interdisciplinary staff)
• Coordinate patient referrals, care plans, and service delivery to ensure optimal outcomes
• Oversee care quality, documentation compliance, and regulatory standards (Medicare/Medicaid)
• Drive performance through team coaching, productivity monitoring, and clinical oversight
• Facilitate interdisciplinary care coordination and case conferences
• Support hiring, onboarding, staff development, and performance management
• Ensure 24/7 patient care coverage and participate in on-call leadership as needed
• Collaborate with leadership on budgeting, quality improvement (QAPI), and operational strategy

• Maintain strong relationships with physicians, community partners, and stakeholders

Qualifications

• Active RN license with clinical leadership experience
• 2+ years clinical experience; 1+ year supervisory experience preferred
• Strong knowledge of home health regulations, Medicare guidelines, and documentation standards
• Proven leadership, communication, and team development skills

• Valid driver’s license and ability to travel locally
