Alpha Home & Health Services is currently seeking an experienced Per Diem Registered Nurse for our Home Health patients in Volusia County, FL.

The Home Health Registered Nurse (RN) Care Manager plays a crucial role in coordinating and managing the care of home health patients. Key responsibilities include:

1. Patient Assessment and Management : The RN Care Manager conducts comprehensive assessments to determine the suitability of home health services for patients and collaborates with physicians to develop individualized home care plans.

2. Education and Support : Educate patients, families, caregivers, and community providers on how to safely deliver care, ensuring that everyone involved understands the care plan and their roles.

3. Interdisciplinary Collaboration : Regular interdisciplinary meetings are conducted to ensure cohesive care delivery, and appropriate referrals to additional services are made as needed.

4. Follow-Up and Evaluation : The RN monitors the effectiveness of the home care plan and the patient/family's response, making adjustments as necessary to achieve desired health outcomes.

5. Quality Improvement Initiatives : The Care Manager identifies areas for performance improvement and participates in designing and implementing programs aimed at enhancing the standard of care in home health settings.

This role requires advanced clinical skills, strong decision-making capabilities, and a commitment to improving patient outcomes through effective care management and collaboration.

Qualifications and Skills

· Must be currently licensed as a RN in the state of FL.

· Minimum of one year nursing experience required.

· Home Health experience strongly preferred.

-Strong clinical skills and effective communication abilities.

-Proficiency in electronic health records.

· Valid FL Driver's License, proof of car insurance and reliable transportation required.

-Level 2 Criminal Background Check.

-CPR certified
