Schedule: Full time Shift: Day (United States of America) Address: 770 W GRANADA BLVD City: ORMOND BEACH State: Florida Postal Code: 32174 Job Description: Schedule: Full Time Days. Monday-Friday 8:30am to 5:00pm Location: Covering Volusia and Flagler Counties. Documents treatment provided in medical records, updating care plans and writing modified orders for changes in the plan of care. Completes all documentation in a timely and accurate manner. Participates in clinical record reviews, agency staff meetings, therapy meetings, and other activities as requested. Provides supervision and guidance to aides, ensuring compliance with care plans and documentation requirements. Performs evaluations and utilizes findings to determine diagnosis (for SLP services), prognosis, and plan of care in consultation with the physician. Determines specific interventions, duration, and frequency needed to achieve anticipated goals and outcomes. Observes and records activities and findings in the clinical record, reporting patient reactions and changes to the physician or primary Case Manager. Assesses patient responses to interventions and performs reassessments as required. Instructs other health team personnel in phases of speech therapy relevant to patient care. Instructs patients and caregivers on speech language improvement programs, including adaptive equipment and home programs. Selects, applies, or modifies skilled interventions to achieve the best patient outcomes. Other duties as assigned. Knowledge, Skills, and Abilities: • Functions with a high degree of independence [Required] • Ability to delegate tasks to appropriate personnel as indicated by skill level and professional standing [Required] • Strong computer and technology skills [Required] • A working knowledge of community resources and an ability to refer patients and families appropriately [Preferred] • Home Care Regulations and Third-Party Reimbursement as it impacts care delivery [Preferred] Education: • Master's speech therapy [Required] Field of Study: • in Speech Language Pathology Work Experience: • 1+ relevant [Required] • 1+ relevant clinical speech therapy experience [Required] • Recent, relevant experience in a Medicare-certified home health agency as a case-manager [Preferred] Additional Information: • N/A Licenses and Certifications: • Speech Language Pathologist (SLP) [Required] • Driver's License (DL) [Required] • Basic Life Support - CPR Cert (BLS) [Required] • Certified for Oasis Specialist-Clinical (COSC) [Preferred] Physical Requirements: (Please click the link below to view work requirements) Physical Requirements - Pay Range: $31.55 - $58.69 This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
