Physical Therapist – Home-Based Care

Are you an experienced Physical Therapist seeking flexibility, autonomy, and the opportunity to make a meaningful impact? We are a clinician-led home therapy provider delivering Physical, Occupational, and Speech Therapy services directly in patients' homes throughout Palm Beach and Broward Counties.

We are currently hiring passionate Physical Therapists to provide exceptional in-home care within their preferred service areas.

Open Opportunities

• Part-Time, PRN, Per Diem, Contract (1099) : Choose your schedule and number of visits (15–35 hrs/week).

• Full-Time W2 Employment : Available in Palm Beach County; includes optional health benefits, PTO, and CEU reimbursement.

Compensation

• Adult 1099 Rates : $70–$90/hour, based on session type (evaluation vs. treatment) and payer. Incentives offered for private pay and commercial patients.

• Pediatric 1099 Rates : $60–$85/hour, paid per unit.

• Full-Time W2 Salary : $75,000–$90,000/year for 30 sessions/week, plus bonus pay for sessions beyond 30/week.

Responsibilities

• Conduct comprehensive evaluations and ongoing assessments in accordance with APTA and state standards.

• Develop personalized Plans of Care and track measurable outcomes.

• Modify treatments based on patient progress and needs.

• Collaborate with patients, caregivers, and healthcare professionals to ensure care continuity.

• Document sessions and maintain accurate records.

• Coordinate discharge planning and follow-up care recommendations.

Qualifications

• Current Florida Physical Therapy License

• Active CPR Certification

• Strong clinical knowledge in PT best practices, biomechanics, and mobility-focused care

• Compassionate, patient-centered approach with the ability to motivate and educate
