Minimum to Midpoint Pay Rate: $31.78 - $42.90 / hour

Summary

Emergency Department Utilization Care Managers perform utilization management functions to ensure medical necessi ty and the appropriate level of care. Assesses, plans, implements, coordinates, monitors, and regulates compliance options and serv i ces to meet an i ndividual's health needs throughout the continuum. Collaborates through communication and available resources to promote quality cost-effective outcomes. Expedites linking patient with systems i n the community in conjunction with ED MSW to provide them with resources, services and opportunities to achieve and maintain maximum functioning. Facilitates concurrent documentation in the electronic medical record to accurately reflect care, services and resources utilized in the delivery of patient care. Successfully meets requirements for productivity and accuracy related to all utilization management processes.
