Home Health Aide

General Summary: Responsible for assisting in Activities of Daily Living (ADLs) and personal care services to residents in accordance with an established care plan, in addition to assisting residents so that independent living can be maintained for as long as appropriate and possible.

Essential Job Duties:

1. Assists residents with dressing, grooming, eating, bathing, positioning, turning, toileting and exercising.
2. Keeps Supervisor informed of residents’ status.
3. Assists with escorts to the dining room, beauty shop, physical therapy, activities, etc.
4. Transport and escort residents as needed.
5. Performs domestic and household tasks.
6. Provide companionship and basic emotional or psychological support.
7. Changes bed linens, makes beds and keeps rooms clean and orderly.
8. Monitors and observes resident’s physical and mental condition.
9. Attends in-service training and education sessions, as assigned.
10. Familiarity with emergency procedures and may be required to assist with evacuations.

Other Duties:

1. Provide medication reminders.
2. Assists the resident with meal service.
3. Assists with ongoing assessment of residents’ needs to promote independence.
4. Maintains accurate records of observations and services rendered to residents.
5. Assists the resident in making doctor’s appointments.
6. Encourages independence, attendance at activities, restorative programs, and rehabilitation.
7. Operate as a member of a team both within the department and across departments.
8. Performs other work duties and responsibilities as assigned.

Qualifications:

1. Ability to communicate effectively and perform simple arithmetic. Ability to understand measurements and conversions.
2. Prior caregiving experience is preferred.
3. Registration with State registry required.
