About the Position

TravelNurseSource is working with Lancesoft Inc to find a qualified Case Manager RN in Cape Coral, Florida, 33904!

Shift

8-Hour Days – 8:00 AM to 4:30 PM

• 40 guaranteed hours per week

• Weekend rotation: 1 weekend per month

• No on-call

• Floating required (travelers float first; may float to sister campuses)

• Facility: Cape Coral Hospital

Key Responsibilities – Case Manager RN (Care Management)

• Perform acute care case management for hospitalized patients

• Complete admission and continued stay reviews

• Coordinate discharge planning (home health, SNF, rehab, DME, etc.)

• Collaborate with physicians, nursing, social work, and interdisciplinary teams

• Ensure compliance with medical necessity and payer guidelines

• Facilitate smooth transitions of care to reduce readmissions

• Document accurately and timely in the EMR

• Function as core staff and integrate into unit culture

• Acute care experience (required)

• Active RN License (must be on profile)

• Nursys verification required

• BLS certification

• Degree accreditation must be on profile OR minimum 5 years of experience
