Overview:

We are seeking a dedicated and experienced Registered Nurse (RN) to serve as a Case Manager within our healthcare organization. This vital role involves coordinating patient care across various settings, ensuring seamless transitions from inpatient to outpatient services, and optimizing health outcomes through comprehensive case management. The ideal candidate will possess extensive clinical expertise, including critical care, trauma, and hospital medicine experience, and demonstrate a strong commitment to patient-centered care.

• RN-Case Manager
• Location: Hudson, FL
• Shift: Days 5x8- 40hrs
• Facility: HCA Florida Bayonet Point Hospital
• Travel Pay: $2206/week

Duties:

• Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations, to determine care needs and develop individualized care plans.
• Coordinate inpatient and outpatient services, facilitating communication among physicians, specialists, therapists, and other healthcare providers.
• Manage discharge planning processes by evaluating post-acute care requirements and arranging appropriate services to ensure smooth patient transitions.
• Oversee utilization management by reviewing diagnostic evaluations, medical records, and treatment plans to optimize resource utilization and maintain compliance with healthcare policies.
