Unity Health and Rehab a 250 bed skilled nursing facility located in Miami, FL, is seeking an experienced Social Services Assistant to join our team. As a Social Services Assistant, you will play a vital role in providing exceptional care to our patients and residents.

About the Role:

• Assist in coordinating patient care and services, ensuring seamless transitions and communication between healthcare providers, patients, and families.

• Provide emotional support and empathy to patients, families, and caregivers, helping to alleviate stress and anxiety.

• Collaborate with the multidisciplinary team to develop and implement individualized care plans, focusing on overall wellness and quality of life.

• Maintain accurate and up-to-date records, reports, and documentation, adhering to regulatory guidelines and company policies.

• Participate in quality improvement initiatives, contributing to the continuous enhancement of our services and care delivery.

Responsibilities:

• Coordinate patient care and services, ensuring timely and effective delivery of services.

• Provide emotional support and counseling to patients, families, and caregivers.

• Participate in care conferences, team meetings, and other collaborative activities.

• Maintain patient confidentiality and adhere to HIPAA guidelines.

• Collaborate with other departments, such as Therapy and Rehabilitation, to ensure holistic care.

• Stay up-to-date with industry developments, best practices, and regulatory requirements.

Requirements:

• Minimum of 2 years of experience in a nursing home setting preferred

• Bilingual English/Spanish preferred

• Possess strong communication, interpersonal, and organizational skills.

• Ability to work effectively in a fast-paced environment, managing multiple tasks and priorities.

• Strong emotional intelligence and empathy, with a passion for supporting patients and families.

• Ability to maintain confidentiality and adhere to HIPAA guidelines.
