About the Position
Conduct comprehensive patient assessments to determine clinical and psychosocial needs.
Develop, implement, and monitor individualized care plans.
Coordinate services across acute care, rehabilitation, home health, and community resources.
Requirements
- Hospital and or case management experience
- Midas, interqual, meditech, experience in acute hospital case managemen
