About the Position

We are seeking an experienced Director of Case Management to lead and oversee all case management operations within the facility. This leadership role is responsible for ensuring effective care coordination, discharge planning, regulatory compliance, and optimal utilization of clinical resources. The Director will collaborate with interdisciplinary teams and executive leadership to drive quality outcomes and operational excellence.

Key Responsibilities

  • Direct and manage all case management department operations
  • Oversee staffing models, productivity, and departmental performance
  • Ensure compliance with organizational policies and regulatory standards
  • Lead care coordination and discharge planning processes
  • Monitor performance metrics and implement process improvements
  • Collaborate with executive leadership including CFO, CMO, and CNO
  • Optimize resource utilization and patient care outcomes
  • Establish strong relationships with internal teams and external agencies
  • Ensure proper documentation and adherence to case management best practices
  • Drive quality improvement and performance initiatives

Requirements

Education

  • Bachelor’s Degree ( required )
  • Master’s Degree in Nursing, Healthcare Administration, or Business ( preferred )

Licensure

  • One of the following required:
    • Registered Nurse (RN) or
    • Licensed Clinical Social Worker (LCSW) or
    • Licensed Master Social Worker (LMSW)

Experience Requirements

  • 3+ years of acute care hospital case management experience (required)
  • 2+ years of leadership experience in case management (required)
  • Proven experience managing departmental operations and interdisciplinary teams

  • Knowledge of healthcare regulations and compliance standards

Benefits

  • Salary Range:
    • $102,107 – $115,000 (0–3 years experience)
    • $115,000 – $127,587 (3+ years)
    • $127,587 – $140,000 (5+ years)
  • Sign-On Bonus: Up to $10,000 (non-HCA candidates)
  • Relocation Assistance: Available
