Case Manager
Overview
We are seeking a compassionate and organized Case Manager to support patients by coordinating care, communicating with providers and interdisciplinary teams, and ensuring timely access to high-quality healthcare services. This role plays a critical part in navigating insurance requirements, connecting patients with appropriate resources, and promoting continuity of care throughout the patient experience.Responsibilities:
- Functions capably in clinical and administrative support positions within the practices
- Works with leadership to identify opportunities and problems and initiates plans for decision-making and problem solving in the organization, and specifically with regards to the practices in a positive manner
- Directly supervises practice staff to include MA’s and any other supportive staff assuring adequate coverage for all position and arranging for coverage for sick and vacation time
- Works with the appropriate staff in the office, assures provider schedules are at a level to achieve identified benchmarks and apprises leadership of any issues regarding achieving benchmark
- Makes recommendations for staffing levels at the site based on site productivity and patient need. Assures adequate provider coverage to meet the patient needs.
- Participates in interviewing, hiring and orientation of new staff at the practices
- Assures employee evaluations are completed in a timely manner and appropriate goals are set for all staff supervised. Makes recommendations for salary adjustments
- Providers oversight of bi-weekly employee timesheets and monitors time activities
- In conjunction with the Leadership team identifies areas of concern for providers and brings forward for resolution and feedback/follow-up
- Works with Leadership to assess improvements made in systems or processes, system efficiency, innovation and creativity as well as commitment to generating new solutions and ideas
Qualifications:
1–2 years of clinical, case management, or medical office experience (medical background preferred)
High School Diploma or GED required
Proficiency with Microsoft Excel and general computer applications
Bilingual in English and Spanish (required)
Strong written and verbal communication skills
Excellent organizational, analytical, and time-management skills
Ability to multitask and problem-solve in a fast-paced environment
Team-oriented with a compassionate approach to patient care
Preferred Skills:
- Bilingual in English and Spanish to serve Miami’s diverse patient population.
- Familiarity with Medicare, Medicaid, and managed care plans in Florida.
- Previous experience in wound care or similar healthcare departments.

Job Types: Full-time, Part-time
Pay: $17.00 - $20.00 per hour
Expected hours: No less than 40 per week
Schedule:
- Monday to Friday from 8:30AM- 5:00PM
Work Location: In person
