Job Overview

We are seeking a strategic and experienced Director of Pharmacy Services to oversee clinical pharmacy operations, medication management, and regulatory compliance across our nursing home facilities. In this leadership role, you will direct our overall pharmacy program, manage relationships with contracted pharmacy providers, and drive clinical cost-containment initiatives. Your focus will be on optimizing resident therapy, maintaining strict regulatory standards, and implementing effective formulary management protocols.

This position requires approximately 25% travel to our various facility locations.

Key Responsibilities

• Strategic Pharmacy Oversight: Lead and direct the pharmacy services program across all facilities, establishing clinical standards, policies, and operational best practices.

• Formulary & Therapeutic Substitution Management: Drive active management of the facility formulary. You will oversee processes for therapeutic substitutions (swapping out medications for safer, more cost-effective alternatives) and ensure strict alignment with preferred drug lists.

• Cost Management & Stewardship: Direct clinical cost-containment strategies, analyze high-cost medication utilization, and establish protocols to reduce pharmacy waste and manage budgets across all locations.

• Regulatory Compliance: Ensure all facilities remain in full compliance with CMS, state, and federal regulations regarding pharmacy services, drug storage, and controlled substances.

• Quality Assurance & Committee Leadership: Lead Pharmacy and Therapeutics (P&T) committee meetings, analyze medication error trends, and establish corrective action plans to improve resident safety.

• Vendor & Partner Management: Oversee performance, service level agreements, and contracts with external pharmacy vendors to ensure timely and accurate medication delivery.

• Staff Training & Clinical Consultation: Provide expert clinical guidance to Medical Directors, Directors of Nursing (DONs), and facility administrators on complex medication regimens, safety protocols, and cost-efficient prescribing practices.

Position Requirements

• Education: Doctor of Pharmacy (Pharm.D.) or Bachelor of Science in Pharmacy from an accredited school of pharmacy.

• Licensure: Active, unrestricted registered pharmacist license in the state of operation.

• Experience: Minimum of 5 years of experience in long-term care (LTC), geriatric pharmacy, or consultant pharmacy operations, with at least 2 years in a leadership or supervisory capacity.

• Clinical Expertise: Proven track record in formulary development, therapeutic substitution protocols, and clinical cost-containment programs.

• Certification: Board Certified Geriatric Pharmacist (BCGP) credential is highly preferred.

• Travel: Ability and willingness to travel up to 25% of the time to conduct site visits, quality audits, and clinical reviews across regional facilities.

• Skills: Exceptional leadership, communication, and analytical skills. Proficiency in electronic health record (EHR) systems and pharmacy billing platforms.
