Mental Health Resource Center is a not-for-profit Florida corporation that provides a wide range of mental health and behavioral health care services to the community. Services include 24-hour emergency services, inpatient psychiatric services for children, adolescents, and adults, as well as outpatient services such as medication management, case management, and counseling. Job Summary
The FACT Case Manager provides treatment, rehabilitation, and support services to individuals served by the FACT Program to support stabilization, reduce hospitalization, and promote successful community living. The Case Manager also promotes recovery, empowerment, self-determination, and active participation in treatment planning for each FACT participant. The FACT (Forensic Assertive Community Treatment) Program is a multidisciplinary clinical team that provides comprehensive, community-based services to adults with severe and persistent mental illness. The program is designed to reduce hospitalization, increase community tenure, and enhance independence, autonomy, and quality of life. FACT team members provide a wide range of services, including advocacy, crisis intervention, assessments, treatment planning, case management, medication support, and coordination of care. Additional services include community referrals, symptom education, vocational and educational support, social skills development, and assistance for families and caregivers. Most services are provided in the community, with approximately 15% of time spent in the office. Company vehicles are available; however, team members must also use their personal vehicles as needed and are eligible for mileage reimbursement. The FACT team operates during and outside of standard business hours, including rotating weekend, holiday, and on-call responsibilities. Responsibilities
- Provides treatment, rehabilitation, and support services in accordance with the FACT model (based on the PACT model).
- Maintains accurate, complete, and confidential clinical records.
- Conducts mental health status assessments.
- Coordinates referrals to community providers and support services.
- Assists individuals with activities of daily living, transportation, communication, and social/interpersonal skills.
- Provides education on mental health symptoms, medications, and recovery.
- Serves as primary case manager for an assigned caseload, promoting recovery, empowerment, and self-determination.
- Reduces time spent on documentation, allowing more focus on client interaction and care
- Transforms brief notes or session conversations into complete, clinically appropriate documentation
- Supports accurate, compliant documentation while allowing staff to review and finalize notes
- Provides helpful insights from sessions to support care planning and continuity
- Accessible on desktop or mobile for flexibility across different work settings
- Bachelors degree in Human Services or a related field required.
- Experience working with adults with severe and persistent mental illness required.
- Proficiency in the RBHS/MHRC Electronic Health Records (EHR) and Patient Information System demonstrated within three months of employment.
- Proficiency in Microsoft Office, Outlook and the use of the Internet.
- Must meet MHRC/RBHS driver requirements, including a valid Florida drivers license, required insurance coverage, and acceptable driving record.
- Ability to work in a variety of settings, including homes, offices, hospitals, nursing homes, and secure units.
- Ability to make sound, independent decisions, including responding to emergencies and high-risk situations.
- Strong communication and interpersonal skills; ability to work effectively with individuals, families, providers, and team members.
