Social Worker-Skilled Nursing

Shift: Shift 1/8:00:00 AM to 4:30:00 PM
Minimum to Midpoint Pay Rate: $25.06 - $32.58 / hour
Summary
Assesses patients that are being transferred from Lee Memorial Health System or from any other appropriate setting to HealthPark Care Center under Medicare or another approved payor source. Assists patients and families with their discharge planning needs. Assists patients and their families to achieve and maintain maximum functioning in and outside the Care Center through needs assessment, crisis intervention, counseling and information and referral. Performs other miscellaneous duties as needed.
