Overview:
We are seeking a compassionate and detail-oriented Registered Nurse (RN) to join our team as a Client Services Coordinator at our upscale retirement community in Wellington, FL. This is a dynamic position that blends clinical oversight with client relationship management and business operations. The ideal candidate is personable, professional, and passionate about providing exceptional care to seniors while supporting the overall coordination of services.
Key Responsibilities:
Serve as the primary point of contact for residents and their families regarding care coordination and service needs.
Collaborate with residents, families, and staff to develop and manage individualized care plans.
Ensure continuity of care by coordinating with internal staff and outside healthcare providers.
Oversee clinical compliance and maintain accurate and up-to-date documentation.
Participate in care plan meetings, assessments, and service reviews.
Support move-in assessments and new resident onboarding.
Monitor resident health changes and intervene when needed.
Assist with scheduling, staffing, and administrative tasks related to client services.
Promote a warm, welcoming environment while balancing clinical excellence with business operations.
Qualifications:
Active Registered Nurse (RN) license in the state of Florida.
Minimum of 2 years of nursing experience, preferably in a senior living, home health, or care coordination setting.
Strong interpersonal and communication skills.
Excellent organizational and time-management abilities.
Knowledge of senior care regulations and best practices is preferred.
Proficiency with EMR systems and Microsoft Office Suite.
