Position Summary

The Referral Coordinator is a bachelor's or master prepared professional with the requisite knowledge, skills and abilities to assist adults with referrals to the most appropriate level of care providers. The Referral Coordinator works as a part of the collaborative care team of healthcare professionals led by a Department of Health primary care provider to support adults with mental health and substance use disorders served in the medical home and assessed and determined to be in need of and will benefit from a level of care that exceeds the low intensity level of care as determined by the LOCUS. The Referral Coordinator has significant knowledge of the community's social services and mental health care resources and works efficiently and effectively with providing appropriate referrals to adults to meet their health care needs. The Referral Coordinator is responsible for tracking the number of adults who require this level of care coordination, and provide a monthly report of clients who are referred out by provider, location and known status of the referral.

Directions for Living is an essential services provider. All Directions for Living Team Members must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation.

EDUCATION EXPERIENCE/REQUIREMENTS:

  • High School Diploma, with at least 2 years of experience in mental health, social services, or healthcare coordination.
  • Familiarity with referral systems and intake processes is a plus.
  • Experience with Avatar required
  • Knowledge of Insurance and funding sources
