Now Hiring: Registered Nurse

Case Manager / MDS coordinator

Full-Time | 4 Months Contract | Competitive Pay

High Chances of Conversion after 4 Months

Location: Ocala, Marion County in FL,

• RN Will Support Five Facilities, 1 Location Per Day,
• These facilities are in Ocala, Eustis, Lady Lake, Mount Dora, and Leesburg

Scheduled: Total 40 hours per week, 5 Days in a week

Shift: M-F 8 am to 5:00 pm, 8 Hours in day.

Hourly Pay: $40.00 - $45.00 on W2

Responsibilities:

• Coordinate comprehensive care plans for residents in collaboration with the interdisciplinary team (physicians, therapists, dietitians, and social workers).
• Conduct initial and ongoing assessments to evaluate residents’ medical, functional, and psychosocial needs.
• Develop, implement, and update individualized care plans based on residents’ changing conditions.
• Monitor residents’ progress and communicate changes in condition or treatment needs to the care team and family members.
• Ensure continuity of care during admissions, transfers, and discharges.
• Serve as liaison between residents, families, healthcare providers, and external agencies.
• Review clinical documentation to ensure accuracy, completeness, and compliance with state and federal regulations.
• Oversee and coordinate utilization review and discharge planning processes.
• Support nursing staff with care coordination, resident education, and complex case management.
• Participate in quality assurance programs, audits, and interdisciplinary care meetings.
• Educate residents and families about care plans, medications, and community resources.
• Ensure compliance with Medicare/Medicaid and managed care documentation standards.

Required Education:

• Must Be state Licensed Nurse.
• Must have BLS/CPR Certificate.
